Release tooling fixes: npm publishing moves to trusted publishing, versions that were tagged but never reached npm are published, and unused TOML config support is removed.

SIP fixes: CANCEL follows its INVITE's path with the matching CSeq, proxy-authenticated dialogs keep the caller's CSeq, `X-Edgeport-Ref` is stripped case-insensitively, `maxContacts: 0` is no longer treated as unlimited, and `fixRequestURI` rewrites the Request-URI user.
